Participation in athletic training, team programs, and related activities involves inherent physical risks. These risks include, but are not limited to, sprains, fractures, concussions, permanent injury, paralysis, death, and exposure to communicable illnesses, including but not limited to influenza and COVID-19. While Watz Academy takes reasonable precautions to create a safe training environment, these risks cannot be fully eliminated.
By signing this waiver, I acknowledge that I fully understand the nature of these risks and freely choose to accept them on behalf of myself and/or my minor child or ward.
I knowingly and voluntarily assume all risks associated with participation in any Watz Academy program, activity, training session, team event, tournament, or related activity, whether such risks are known or unknown, and whether arising from the negligence of Watz Academy or otherwise.
I agree to take full responsibility for my participation, or the participation of my minor child or ward, and will not hold Watz Academy or any affiliated party responsible for any injury, illness, or loss that may occur.
If at any time I observe a condition or situation I believe to be unsafe, I agree to remove myself or my child from participation immediately and notify Coach Watson or program staff as soon as possible.
I, on behalf of myself, my minor child or ward, and our respective heirs, assigns, personal representatives, and next of kin, hereby release, waive, and hold harmless Watz Academy, its founder and director Devin Watson, coaches, volunteers, staff, agents, partner facilities, and any affiliated individuals or organizations (collectively the "Released Parties") from any and all claims, demands, losses, damages, or liability arising from injury, illness, disability, death, or loss or damage to person or property, whether arising from the negligence of the Released Parties or otherwise, to the fullest extent permitted by law.
I hereby grant Watz Academy permission to photograph and record video and audio of myself and/or my minor child or ward during training sessions, practices, games, tournaments, and related program activities. I understand that Watz Academy is free to utilize this material for promotional, educational, and marketing initiatives across its website, social media platforms, and other avenues, with no restrictions on location or time frame.
I waive the right to inspect or approve the finished content in which my or my child's likeness appears, and I waive any right to compensation or royalties related to the use of such content. I understand I will not be consulted prior to the use or publication of any photograph or recording.
If I do not wish for myself or my child to be included in any photography or video recording, I understand I must notify Watz Academy in writing at watzacademy@gmail.com prior to the first session.
I confirm that the person named below is in sound physical condition and fit to engage in all Watz Academy program activities. I also agree to notify Watz Academy personnel of any pre-existing medical issues, injuries, or physical limitations that could affect their safe participation before training begins.
I confirm that I am the parent or legal guardian of the minor named below and am authorized to sign this waiver on their behalf. I have gone over the contents of this waiver with my child, including the risks involved and expectations for conduct and participation, and my child understands and accepts these terms.
On behalf of myself, my spouse, and my minor child or ward, I agree to release, indemnify, and hold harmless the Released Parties from any and all claims, liabilities, or damages arising from or related to my child's participation in any Watz Academy program or activity, including those arising from the negligence of the Released Parties, to the fullest extent permitted by law.
I pledge to adhere to all of Watz Academy's regulations, standards, and expectations concerning behavior, attendance, and safety. I acknowledge that not complying with this could lead to my dismissal from the program, and I wouldn't get my money back.
This waiver is governed by California law and is intended to be valid and enforceable to the fullest extent that law permits. This agreement is entered into in San Diego County, California. If any provision of this waiver is found to be unenforceable by a court of competent jurisdiction, the remaining provisions will remain in full effect.
By signing below, I acknowledge that I've read this waiver completely. I understand what it says, including the rights I'm giving up. I'm signing it of my own free will, without anyone pushing or offering anything in return.
